I have completed and sent in the questionnaire. Why am I receiving a reminder letter?
If you have sent in the questionnaire via the internet, a confirmation of receipt will be displayed on your screen instantly. Did you not receive a confirmation on your screen? Please check that you did not accidentally press the 'Save' button instead of the 'Send' button. Log in again (if necessary), go to the last screen, and press 'Send'. If you have completed and submitted the questionnaire recently, your questionnaire and the reminder letter may have crossed.
