I have completed and submitted the questionnaire. Why have I been sent a reminder?

When you submit the questionnaire online, a confirmation of receipt will immediately appear on your screen. If you did not see this on-screen confirmation, please check that you clicked ‘Send’ and did not accidentally click ‘Save’ instead. Log in again if necessary, go to the final screen and click the ‘Send’ button. If you only recently completed and submitted the questionnaire, your questionnaire and the reminder may have crossed in the mail.
